Got HTTP 404 error when upload file ?

Hi experts,
I've got the HTTP 404 - File not found when doing the file attachment in prod server but not in dev server.
I'm using Oracle 11.5.9 (Linux AS 2.1) with the same configuration between both prod and dev.
Could someone tell me how to solve this problem.
